Типовая форма заявления о включении в список получателей льгот

Министерство финансов утвердило типовую форму Заявления о включении экономического субъекта в список предприятий и организаций объединений слепых, объединений глухих и обществ инвалидов, освобожденных от уплаты НДС без права вычета при импорте сырья, материалов, комплектующих изделий и принадлежностей, необходимых для производства продукции, а также освобожденных от уплаты в бюджет НДС за произведенные товары и оказанные услуги. Это заявление подается в соответствии с положениями Постановления Правительства № 819 от 18 октября 2017 г. об утверждении Положения о порядке предоставления некоторых категорий льгот по уплате НДС согласно положениям ч. (18) ст. 4 Закона № 1417-XIII от 17 декабря 1997 г. о введении в действие раздела III Налогового кодекса, с последующими изменениями и дополнениями. В заявлении указывается полное название общества учредителя/участника и его фискальный код. Также представляется информация об объединении слепых/ глухих и/или общества инвалидов, зарегистрированных в качестве некоммерческих организаций в Агентстве госуслуг (номер, дата и серия свидетельства). Также заполняются данные сертификата, подтверждающего общественную пользу общества (выдается Минюстом). Заявитель должен указать численность лиц с ограниченными возможностями, работающих в организации или на предприятии, по каждому календарному месяцу в отдельности, а также их долю в общем числе сотрудников. Также представляется информация о среднемесячной зарплате по предприятию или организации, начисленной и выплаченной зарплате сотрудникам-лицам с ограниченными возможностями. В качестве подтверждения использования труда лиц с ограниченными возможностями в процессе производства/предоставления услуг заявитель может представить описание проделанной работы, продуктов/услуг, которые планируется выполнить с учетом льготы; описание технологических процессов; указание должностей, занимаемых на предприятии лицами с ограниченными возможностями. Стоит отметить, что к типовому заявлению на получение льготы прилагается пакет документов (копия сертификата о регистрации некоммерческой организации, копия сертификата о присвоении статуса общественной пользы, копия свидетельства о регистрации предприятия или организации, копия последнего финансового отчета, представленного в Национальное бюро статистики и др.). Приказ Минфина опубликован в «МО» и вступил в силу 18 января 2019 г.
