Serviciul electronic e-CNAM

În atenţia angajatorilor şi instituţiilor responsabile de prezentarea formularelor 2-03/l, 2-04/l Compania Naţională de Asigurări în Medicină informează despre disponibilitatea serviciului de raportare on-line a listelor de evidenţă nominală a persoanelor asigurate angajate (formularul 2-03/l) şi listelor de evidenţă nominală a persoanelor asigurate din contul statului (formularul 2-04/l). Pentru a putea utiliza serviciul e-CNAM, este necesar de a parcurge următorii paşi: 1) Activaţi semnătura digitală:
  • prin telefon, care poate fi obţinută la unul din operatorii de telefonie mobilă Orange sau Moldcell;
  • prin cartelă, care poate fi obţinută de la Centrul de Telecomunicaţii Speciale.
2) Accesaţi site-ul www.raportare.md ori www.servicii.gov.md:
  • înregistraţi pe portal persoana responsabilă de expedierea formularelor 2-03/l, 2-04/l din numele instituţiei pe care o reprezentaţi;
  • accesaţi profilul utilizatorului pentru a alege serviciul e-CNAM şi adăugaţi organizaţia din numele căreia veţi depune formularele 2-03/l, 2-04/l;
  • descărcaţi şi completaţi cererea de solicitare a autorizaţiei pentru prezentarea formularelor 2-03/l, 2-04/l prin serviciul e-CNAM, aplicaţi ştampila instituţiei pentru a o prezenta la Agenţia teritorială la care sunteţi arondaţi;
  • după validarea dreptului de reprezentare în sistem, treceţi în biroul personal, alegeţi serviciul e-CNAM, completaţi şi expediaţi formularul de raportare cu aplicarea semnăturii digitale.

Anexe: Cerere de solicitare a autorizaţiei pentru prezentarea formularelor. Prezentare eCNAM Notă: Lista de evidenţă nominală a persoanelor asigurate angajate (formularul 2-03/l) şi lista de evidenţă nominală a persoanelor asigurate din contul statului (formularul 2-04/l) trebuie să fie prezentate pînă la data de 7 a lunii următoare celei în care au avut loc schimbările (pct. 9 din HG nr. 1015 din 05.09.2006).
